COPD Patient experiences needed for survey

Your local NHS wants to improve how you are supported to manage your COPD and are collecting feedback from patients and carers about the support you receive for COPD in your area. Your views and experiences will be used to improve the care you receive from your local NHS. We would be grateful if you can share your thoughts and experiences.

This survey is for anyone with a diagnosis of COPD, or can be completed on behalf of someone with a diagnosis of COPD.  We want to hear patient experiences of pulmonary rehab and explore digital access and literacy.

The patient survey is online, but can be completed over the phone if the patient does not have access to the Internet. For patients who complete the survey, we will send out a £5 Costa voucher to say thank you. The survey is open to anyone with COPD who is resident in the UK.  By providing your feedback, you will be helping your local NHS to understand the needs of people with COPD, so that we can improve our services and support for you and others.
