0.6wte – 22.5hours over 3days (this may increase to full time on development of service)
Initial 1 Year fixed term (which will be reviewed with a possible extension)
Salary – £24,500 per annum
The newly introduced NHS Diabetes Prevention Programme is being implemented in identified sites across England to try and reduce the ever increasing demand of type 2 Diabetes patients. St Helens and Sefton are early implementation sites.
Living Well Taking Control is one of four providers delivering the National Diabetes Prevention Programme across England. We are using our own evidence based programme to support people through Lifestyle change across a 12 month period. The aim is to support people to make sustainable changes that help them to take control of their lifestyles, diet and to better manage their health. Key to the success of this programme will be the Co-ordinator role, working closely with GP Practices, other clinical stakeholders as well as community leaders that can help influence take up of the service.
Therefore, we are looking for an experienced and motivated person who can work under their own initiative and to prioritise a complex range of tasks and able to work flexibly. The Coordinator will be a good communicator and have the ability to lead a team of wellbeing facilitators.
The Co-ordinator post will cover both St Helens and Sefton (must be a car owner -travel will be reimbursed)
Secondment may be considered under certain circumstances.
