Wellbeing Facilitator (Bank Staff) Opportunity

Job Purpose:

To motivate and support participants of the National Diabetes Prevention Programme to take control of their lifestyle management through a 12month programme. To achieve high rates of retention to the programme and help participants to understand their risk of and the implications of Diabetes, and to achieve weight loss and reduced blood sugar levels (HbA1c). The Living Well Taking Control Diabetes Prevention Programme will support and motivate participants to make positive changes to their life by raising their confidence and empowering them to make relevant modifications to their diet and levels of physical activity.

Salary:                      £12.00 per hour for delivery and necessary administration

Hours:                      According to demand and in line with programme time

‘allowances’ as defined.

Location:                 Within agreed and defined areas of Sefton

KEY DUTIES & RESPONSIBILITIES

 Group Education

  • To deliver group education sessions through proactive facilitation and encouraging participation of group members and enhance peer support
  • To facilitate and support groups to make sustainable behaviour changes by motivating and increasing confidence during the change process.
  • To encourage groups of individuals to reflect on their own lifestyle choices and facilitate positive change
  • To take a person centred approach in a group to encourage patients-service users to set goals and action plans in relation to nutrition, physical activity and positive wellbeing.
  • To facilitate health related discussion between individuals in groups
  • To use a varied approach in the delivery of group sessions based on the patients-service users
  • To use suitable tools to support a variety of learning styles
  • To perform point of care blood testing for HbA1c (with training)
  • To ensure that culturally sensitive resources are used in all of the programmes where deemed necessary and accepted and understood by the group participating
  • To ensure that patients/service users get a locally based service and at a time and venue that is convenient to them ensuring that there are no barriers to them participating on the Diabetes Prevention Programme

 

Service Delivery 

  • To work within the systems around the delivery of the programme ensuring that all aspects of communication are used to contact participants pre and post course
  • To work to all quality standards outlined in the service delivery framework ensuring that the programme maintains a high quality level
  • To support the continuous improvement to the quality of the service by participating in regular audit procedures
  • To maintain accurate and complete records pertaining to the course participants and programme
  • To ensure the storage of personal data is kept in strict adherence to maintaining confidentiality and the requirement of the Data Protection Act 1998
  • To be aware of local community groups and signpost to relevant services where appropriate/health identified

Essential Criteria:

KNOWLEDGE:

  • Educated to Diploma level or equivalent experience to match
    • qualification level
  • Knowledge and understanding of the principles of self- management and motivational interviewing.

SKILLS:

  • High ability to communicate with individuals at all levels and using various forms of communication
  • IT Literate
  • High Level of organisational ability
  • Ability to facilitate and engage with participants
  • Accurate report writing skills and the ability to report weekly activity to programme manager

EXPERIENCE:                                          

  • At least 2 years’ experience in a community involvement role.
  • Experience of motivational delivery
  • Experience of facilitating group sessions
  • Worked with a variety of communities, health care professionals and patients in a health promotion environment

PERSONAL ATTRIBUTES:

  • Demonstrate drive, energy and enthusiasm for self-management principles, health promotion and long term conditions
  • Ability to motivate self and course participants
  • Assertiveness in new environments
  • Car Driver

Desirable Criteria:

KNOWLEDGE and Experience:

  • Degree in health related topic – health & social care, sports science, sports therapy, psychology, care, etc
  • Have used motivational interviewing techniques before with patient’s/service users
  • General knowledge on diabetes and its impact on both the individual and wider community
  • Knowledge of local area population demographics and Community provision

 

EXPERIENCE:

  • Motivational Interviewing (MI)
  • Working with BME groups
